This is an exciting opportunity to join us in contributing to our mission by providing clinical governance and oversight for our emerging Home Care Package program in Inner South-West Sydney, based out of our Rockdale office. This role will see you working Monday - Friday business hours, with occasional work required outside these hours,

Changing the Face of Ageing
You will join a small but growing team committed to Changing the Face of Ageing and working closely together to support each person within the HCP program effectively . This is not a clinical role. Instead, you will focus on providing clinical oversight and clinical governance of appropriate health and support services for each client. Your day-to-day will include:
Case Setup – Meet with prospective clients and their families to understand their goals and current needs and develop and implement a care plan to meet those goals and needs. Clearly and confidently communicate the details of the Home Care Agreement to the prospective client.
Case Management – Conduct regular quality assurance visits with clients to ensure a high level of care is being attained. Working closely with clients to manage their HCP budget as efficiently as possible.
Collaboration – Coordinate closely with approved service providers, our office staff, our caregivers, clients and their families to ensure each client’s unique needs are being met.
This role will involve car travel within the local community as required.
Your Skills
You will be a Registered Nurse with significant proven HCP experience in the home care sector and have a passion for providing high-quality care to clients. Experience and knowledge of the Home Care Package program is essential.
You will be empathetic, caring and enthusiastic about moving into clinical oversight and care coordination with an organisation that will commit to your long-term success with us.
You will also bring or be:
- AHPRA registration as a registered nurse
- Full Vaccination against COVID-19
- Comfortable undergoing a Police Check
- An unrestricted Driver’s License
